Croeso i'r rhifyn diweddaraf o Pharmac-e, bwletin newyddion Bwrdd Fferylliaeth Cymru Cymdeithas Fferyllol Frenhinol Prydain Fawr. Mae'r misoedd diwethaf wedi bod yn rhai prysur iawn i ni wrth i ni gynrychioli'r proffesiwn fferyllol yn ymgynghoriadau pwysig Llywodraeth Cynulliad Cymru ar ad-drefnu'r Gwasanaeth Iechyd Cenedlaethol yng Nghymru, ac wrth i ni barhau hefyd â'r gwaith allweddol o sicrhau bod meddyginiaethau diogel yn rhan greiddiol o'r ddarpariaeth fferyllol ledled Cymru.
R'yn ni hefyd wedi chwarae rhan flaenllaw yn y broses heriol o drawsnewid Cymdeithas Fferyllol Frenhinol Prydain Fawr, wrth i ni agosáu at sefydlu Corff Arweiniad Proffesiynol newydd i'r proffesiwn fferyllol. R'yn ni'n cefnogi hefyd ddatblygiad corff rheoleiddio newydd. Y nod ar hyn o bryd yw cyflawni'r newidiadau hyn erbyn Gwanwyn 2010 a gallwn sicrhau'n rhan-ddalwyr ein bod yn gweithredu i sicrhau bod y proffesiwn fferyllol yn parhau i gael ei ddatblygu, ei gynrychioli a'i reoleiddio ledled Prydain. Byddwn yn rhoi gwybod i chi am unrhyw ddatblygiadau pellach wrth i'r broses hon fynd yn ei blaen.
Bydd y misoedd nesaf yn gyfnod o newid i'r Gwasanaeth Iechyd Cenedlaethol yng Nghymru, ac fe fyddwn ni'n cefnogi'r newid hwnnw trwy weithio i sicrhau bod cyngor fferyllol ar gael ymhob rhan o'r Gwasanaeth Iechyd, yn arbennig felly yn ystod y cyfnod o newid sydd i ddod.

Cyfraniad fferylliaeth i feddyginiaethau diogel |

Mae'r Gymdeithas Fferyllol Frenhinol yn benderfynol o wireddu'i nod o sicrhau bod Prydain yn un o'r lleoedd mwyaf diogel yn y byd i dderbyn meddyginiaethau. Mewn adroddiad diweddar, cyflwynodd RPSGB y wybodaeth ddiweddaraf am ddiogelwch meddyginiaethau ac fe nodwyd sawl argymhelliad allweddol er mwyn sicrhau defnydd mwy diogel a chymwys o feddyginiaethau.
Mae diogelwch y claf yn un o flaenoriaethau allweddol y Gwasanaeth Iechyd yng Nghymru ac mae diogelwch meddyginiaethau yn rhan allweddol o hynny. Mae Ymgyrch 1000 o Fywydau yn ceisio lleihau peryglon i gleifion ac yn cydnabod y ffaith bod rheoli meddyginiaethau yn ffactor hollbwysig wrth fynd ati i geisio atal 1,000 o farwolaethau diangen a 50,000 achos o niwed yng Nghymru dros y ddwy flynedd nesaf. Mae niwed o'r fath yn cynnwys sgil-effeithiau o ganlyniad i feddyginiaethau a sgil-effeithiau'n deillio o'r defnydd o feddyginiaethau – gan bobl broffesiynol y diwydiant iechyd a chan y cleifion eu hunain.
Mae adroddiad RPSGB, The Contribution of Pharmacy to Making Britain a Safer Place to take Medicines, yn galw am gynllun gweithredu pum i ddeng mlynedd, wedi'i greu gan gorff arweiniol newydd y proffesiwn fferyllol. Mae yna alw hefyd ar i ddarparwyr gofal iechyd weithio ar y cyd i wella diogelwch meddyginiaethau ac i'r proffesiwn fferyllol weithio'n agosach â sefydliadau iechyd a grwpiau cleifion eraill er mwyn gyrru'r gwaith pwysig hwn yn ei flaen.

Gwella argaeledd meddyginiaethau i gleifion yng Nghymru |

Mae Bwrdd Fferylliaeth Cymru'n croesawu adroddiad Yr Athro Routledge, 'Towards improving the availability of medicines for patients in Wales'.
Mae'r adroddiad yn cynnwys nifer o'r materion a godwyd gan Fwrdd Fferylliaeth Cymru pan gyfrannodd i waith y Grŵp Adolygu Arbenigol. Mae'r rhain yn cynnwys cyflymu'r broses werthuso, chwyldroi trefniadau er mwyn gwella argaeledd meddyginiaethau a ddefnyddir ar ddiwedd bywyd person nad ydyn nhw ar hyn o bryd yn ateb criteria y Sefydliad Cenedlaethol er Rhagoriaeth Clinigol a Iechyd (NICE) o ran eu cost-effeithiolrwydd, a sefydlu cynlluniau rhannu risg a chost gyda'r diwydiant fferyllol.
Un pwnc a godwyd gan Fwrdd Fferylliaeth Cymru ac a ystyriwyd gan y Grŵp Adolygu, oedd prynu'n breifat feddyginiaethau nad ydyn nhw ar gael gan y Gwasanaeth Iechyd. Mae'r Bwrdd o'r farn y dylid cyfyngu gwasanaeth o'r fath gan ganolbwyntio yn hytrach ar wella argaeledd meddyginiaethau effeithiol i gleifion y Gwasanaeth Iechyd. Mae'r Bwrdd yn credu'n gryf y bydd angen, mewn achosion lle nad yw meddyginiaethau ar gael gan y Gwasanaeth Iechyd, rhoi cyngor proffesiynol diduedd i gleifion fydd yn ystyried prynu meddyginiaethau yn y sector preifat. Rôl fferyllwyr yn hyn o beth fydd cynghori cleifion yn y materion hyn a'u cyfeirio nhw at ffynonellau gwybodaeth dibynadwy.
Mae Bwrdd Fferylliaeth Cymru hefyd yn croesawu penderfyniad y Gweinidog Iechyd a Gwasanaethau Cymdeithasol i sefydlu grŵp fydd yn ystyried yn fanylach y cwestiynau sydd yn ymwneud â chost prynu meddyginiaeth ychwanegol (“top-up”) yng Nghymru. |

Rhybudd RPSGB am beryglon meddyginiaethau ffug |
 |
Yn rhan o ymrwymiad RPSGB i leihau achosion o niwed a gwella iechyd cleifion, mae RPSGB wedi dechrau cydweithio ag Asiantaeth Reoleiddio Meddyginiaethau a Chynhyrchion Gofal Iechyd (MHRA) er mwyn tynnu sylw at y peryglon sydd yn deillio o gam-labelu meddyginiaethau – boed hynny ar ddamwain neu'n fwriadol – a meddyginiaethau sydd yn cynnwys y cynhwysion anghywir.
Mae meddyginiaethau ffug yn peri risg cynyddol i iechyd cyhoeddus wrth i fwy a mwy o feddyginiaethau o'r fath gael eu gwerthu ar-lein a thrwy ffynonellau rhyngwladol eraill. Cynhyrchwyd taflen wybodaeth i hysbysu'r cyhoedd am y peryglon sydd yn gysylltiedig â'r meddyginiaethau hyn. Caiff y pamffled maint cerdyn post hwn ei ddosbarthu i bob fferyllfa ym Mhrydain dros y deufis nesaf, ac fe fydd yn cynnwys cyngor ymarferol yn ymwneud â pheryglon meddyginiaethau ffug, sut i leihau'r risg o brynu meddyginiaethau ffug a chyngor ar weithredu os ydych chi'n amau eich bod wedi derbyn meddyginiaethau ffug.
Llynedd, lansiodd RPSGB logo fferyllfa ar-lein (gweler y llun), er mwyn galluogi i ddefnyddwyr adnabod fferyllfeydd bona fide ar-lein. Mae'r logo'n dynodi bod y fferyllfa'n cael ei rheoli a'i gweinyddu gan fferyllydd sydd ar gofrestr RPSGB; mae hyn yn fodd o ddadwneud y peryglon posib o brynu meddyginiaethau ar-lein gan fferyllfeydd nad ydyn nhw wedi'u rheoleiddio'n broffesiynol.

Ebrill 27ain 2009 – Symposiwm Rhag-nodi Anfeddygol
Caiff digwyddiad un-diwrnod, wedi'i drefnu ar y cyd gan Fwrdd Fferylliaeth Cymru RPSGB a'r Coleg Nyrsio Brenhinol (Cymru), ei gynnal yng Nghaerdydd ar ddydd Llun, Ebrill 27ain, er mwyn tynnu sylw at enghreifftiau o rag-nodi anfeddygol llwyddiannus ac effeithiol yng Nghymru.
Bydd y digwyddiad hwn, y cyntaf o'i fath yng Nghymru, yn gyfle unigryw i fferyllwyr a nyrsys sydd yn rhag-nodi ar ran cleifion archwilio gyda'i gilydd y datblygiadau yn eu gwahanol swyddi a'r heriau sydd yn codi o ddydd i ddydd. Gan dynnu ar brofiadau personol, bydd y digwyddiad hefyd yn dangos sut y mae rhag-nodi anfeddygol yn cael ei ddefnyddio ledled Cymru, a'r cyfraniad cadarnhaol a phwysig sydd yn deillio o hynny i ofal cleifion a'r ddarpariaeth o wasanaethau iechyd.
25th – 30th Mai 2009: Eisteddfod yr Urdd
Wedi'i gynnal y tu allan i adeilad y Senedd ym Mae Caerdydd, Eisteddfod yr Urdd yw gŵyl ddiwylliannol ieuenctid mwyaf Ewrop. Bydd gan Fwrdd Fferylliaeth Cymru RPSGB stondin ar faes yr Eisteddfod, i roi gwybodaeth i bobl ifainc am yrfaoedd ym myd fferylliaeth ac i hyrwyddo rôl RPSGB fel cefnogwr a datblygwr y proffesiwn yng Nghymru. |

Lleihau gwastraff o feddyginiaethau |

Mae gwastraffu meddyginiaethau yng Nghymru yn costio £56miliwn i'r Gwasanaeth Iechyd Cenedlaethol bob blwyddyn – mae hynny'n gyfwerth â 10% o holl gyllideb rhag-nodi GIC Cymru.
Mae hyn yn broblem sylweddol, felly, ac mae GIC a Bwrdd Fferylliaeth Cymru RPSGB yn awyddus i gefnogi ffyrdd o leihau gwastraff o'r fath yng Nghymru. Fel arbenigwyr ar feddyginiaethau, mae gan fferyllwyr rôl hollbwysig i'w chwarae wrth fynd ati i leihau gwastraff. Gall fferyllwyr gyfrannu trwy helpu pobl i ddeall eu meddyginiaethau a'u hannog nhw i archebu meddyginiaethau angenrheidiol yn unig. Bydd y cyngor hwn yn fuddiol i gleifion unigol a, thrwy leihau gwastraff, gall y Gwasanaeth Iechyd Cenedlaethol ddefnyddio'r arian gaiff ei arbed i drin mwy o gleifion.
Ym mis Mawrth eleni, roedd fferyllwyr yn Sir Gaerfyrddin, Ceredigion a Sir Benfro yn rhan o ymgyrch i leihau gwastraff o feddyginiaethau yn ardaloedd eu Byrddau Iechyd Lleol; yn ôl yr amcangyfrifon, roedd gwastraff o'r fath yn cyfrannu at gost o ryw £1.5miliwn ymhob un o'r Byrddau Iechyd Lleol hyn. Nod yr ymgyrch yng ngorllewin Cymru oedd codi ymwybyddiaeth ymysg cleifion ac aelodau'r cyhoedd o bwysigrwydd rheolaeth effeithiol o feddyginiaethau er mwyn osgoi gwastraff. Anogwyd cleifion hefyd i ofyn am gyngor gan eu fferyllydd cyn archebu meddyginiaethau – er mwyn sicrhau bod y meddyginiaethau hynny'n wirioneddol angenrheidiol.

Welcome to the latest edition of Pharmac-e, the bulletin that brings you news from the Welsh Board of the Royal Pharmaceutical Society of Great Britain.
The past few months have certainly been a busy time for us in voicing the pharmacy’s profession opinion to the important Welsh Assembly Government consultations for reorganising the NHS in Wales, and crucially in continuing to ensure that medicine safety remains paramount in the provision of pharmacy services and advice across Wales.
We have also been actively engaged in supporting the challenging process of change for the Royal Pharmaceutical Society of Great Britain as we move ever closer to establishing a Professional Leadership Body for pharmacy plus supporting the Governments development of a new regulatory body for the profession. We are on course for these changes to take place in Spring 2010 and our key stakeholders can be assured that considerable steps are being taken to ensure that the profession continues to be effectively developed, represented and regulated across the United Kingdom. We will keep you updated as this process develops.
The next few months will be a period of change for the NHS in Wales, one which we will continue to support through our endeavours of ensuring that access to pharmaceutical advice is available throughout the NHS organisational structures, especially during the transitional time ahead.

The contribution of pharmacy to medicines safety |

RPSGB is committed to achieving its ambition of establishing Britain as the safest place in the world to receive medicines. In a recent report, RPSGB set out what is known about medicines safety and has made a number of key recommendations to help improve the safe and appropriate use of medicines.
Patient safety is a key priority for the NHS in Wales and medicines safety is a key aspect of this. The 1000 Lives Campaign which aims to reduce risks to patient safety recognises the importance of medicines management as a key aspect in the drive to prevent 1,000 avoidable deaths and 50,000 episodes of harm across Wales over the next two years. Preventable harm includes both the adverse effects of the medicines themselves and issues arising from how they are used by both healthcare professionals and patients.
The RPSGB report, The Contribution of Pharmacy to Making Britain a Safer Place to take Medicines, calls for a five to ten year action plan to be drawn up by the new professional leadership body for pharmacy, working in collaboration with other key healthcare providers to improve medicines safety. It also calls for the pharmacy profession to work more closely with other health organisations and patient groups in taking this important work forward.

Improving access to medicines for patients in Wales |

The Welsh Board of RPSGB has welcomed the publication of Professor Routledge’s Report Towards improving the availability of medicines for patients in Wales.
The report incorporates several of the issues raised by the Welsh Board in its submission to the Expert Group Review. These include speeding up the appraisal process, reforming procedures so as to improve the availability of medicines used near the end of life that do not meet current cost effectiveness criteria as set out by the National Institute for Health and Clinical Excellence (NICE), and the establishment of risk and cost-sharing schemes with the pharmaceutical industry. One of the key issues raised by the Welsh Board and taken into consideration by the Expert Review group concerns the issue of privately purchasing medicines not available on the NHS. The Board believes that the need for ‘topping-up’ NHS care in this way should be minimised in Wales as far as possible and that the focus of attention should instead be on improving access to effective medicines for NHS patients. The Board firmly believes that in circumstances where medicines are not available under the NHS, there will be a need to provide unbiased professional information to any patient who might be considering accessing those medicines privately. The role of pharmacists will be crucial in advising patients on these issues and in signposting them to reliable and trustworthy sources of information in these situations.
The Welsh Board has also welcomed the decision by the Minister for Health and Social Services to establish a group to consider in further detail the issue of top-up fees for medicines in Wales.

RPSGB warn of the dangers of counterfeit medicines |

As part of its commitment to reduce harm and improve patient safety, RPSGB has teamed up with the Medicines and Healthcare products Regulatory Agency (MHRA) to highlight the dangers of counterfeit medicines such as those medicines which are deliberately and fraudulently mislabelled and those which include incorrect ingredients. Counterfeit medicine is posing an increasing risk to public health as the amount being sold online and through other sources grows internationally. An information leaflet aimed at the general public has been produced to warn of the dangers. The new double-sided post-card sized leaflet will be distributed to all pharmacies in Great Britain over the next two months, offering practical advice about the dangers of counterfeit medicines, how to minimize the risks of purchasing them and what to if members of the public suspect they have been supplied with them.
Last year, RPSGB launched the internet pharmacy logo (pictured) to help the public identify bona fide internet pharmacies. The presence of the logo indicates that the website is operated by a pharmacy registered with RPSGB and so helps counter the potential dangers to the public of purchasing medicines over the internet from unregulated outlets.
The new patient post-cards perfectly complement the guidance on counterfeit medicines for pharmacists and dispensing doctors that has already been produced through the collaboration of RPSGB, MHRA and the Dispensing Doctor’s Association (DDA).

27th April 2009 – Non Medical Prescribing Symposium
A one day event jointly organised by the Welsh Board of RPSGB and the Royal College of Nursing (Wales) will be held on Monday 27th April in Cardiff to highlight good practice in non-medical prescribing in Wales.
This event, the first of its kind in Wales, will provide a unique opportunity to bring together pharmacist and nurse prescribers to explore the development of their roles and identify the challenges they face in their daily practice. Drawing on their experiences, this event will also highlight examples of how non-medical prescribing is being used across Wales and how it is enhancing both patient care and service delivery.
25th – 30th May 2009: National Urdd Eisteddfod
Held just outside the Senedd in Cardiff Bay, the Urdd Eisteddfod is the largest cultural youth festival in Europe. The Welsh Board of RPSGB will have a stall on the Eisteddfod ‘maes’ to provide information to young people on careers in pharmacy, reflecting the role of RPSGB in promoting and developing the profession in Wales.

Reducing medicines waste |
 |
Wasted medicines are costing the NHS in Wales around £56million each year – that’s equivalent to 10% of the NHS Wales prescribing budget. This is a significant problem for the NHS and the Welsh Board of RPSGB is keen to support positive moves across Wales to help reduce medicines wastage.
As the experts in medicines, pharmacists have a crucial role to play in reducing medicines waste. Helping people understand their medicines and supporting them to only order the items they need are key ways in which pharmacists can help. Not only will this advice help individual patients but by reducing waste, the NHS can utilise the money saved to treat more people.
In March this year, pharmacists in Carmarthenshire, Ceredigion and Pembrokeshire were at the centre of a month-long campaign to reduce medicines waste across their Local Health Board areas, where it was estimated that waste was costing the three LHBs around £1.5million each year. The campaign in West Wales aimed to raise awareness among patients and the general public of good management of medicines so as to avoid waste. Patients were also encouraged to ask for advice from their pharmacist before ordering any medicines they may not need. LINK: Camarthenshire LHB Press Release
